Integrated Referral at the Point of Care
Primary care teams — including physicians, nurse practitioners, and physician assistants — refer patients to the program when prescribing a medication, such as a GLP-1, for chronic conditions like obesity.
Seamless Patient Enrollment & Support
Patients join the program online, provide consent to share their health data, and complete validated patient reported outcomes (PRO) surveys. Patients are also offered access to the Mayo Clinic Diet, an accredited app featuring a comprehensive, whole-health menu program developed and approved by Mayo Clinic.


Ongoing Data Collection & Insights
Through an IRB-approved protocol, Omnia collects data directly from patients (surveys) and from participating healthcare systems (EHR data), as well as health app usage, creating a long-term view of medication adherence, clinical outcomes, and patient experience — all linked to real-world care patterns.
Continuous Practice Engagement & Feedback
Referring practices stay connected through periodic data summaries and benchmarking reports, showing how their patients’ experiences compare to national trends. Providers also receive tools and education, with a focus on more complex therapies, as well as evolving insights into treatment patterns and real-world outcomes — helping them optimize patient care.
